Hermes Manual Approval / Hermes 人工审批

人工审批消费原语：HermesManualApproval.psm1 以审批 SHA-256 为幂等键记录 fixture_record 人工启动审批的一次性消费（重复消费抛错），拒绝负预算，消费记录写入 _security/fixture_record_approvals/，为受监督 Worker 的人工放行流程提供证据。

Manual-approval consumption primitives: HermesManualApproval.psm1 records one-shot consumption of fixture_record manual-start approvals keyed by the approval SHA-256 (re-consumption throws), rejects negative budgets, and persists consumption records under _security/fixture_record_approvals/, providing evidence for supervised-worker release flows.

• 幂等消费：同一审批哈希仅可消费一次 / Idempotent consumption keyed by the approval SHA-256
• 预算校验：拒绝负预算 / Negative-budget rejection
